The administration of large dose IV Vit C (IVC) in the treatment of acute pancreatitis.

In a group of 39 patients diagnosed of severe acute pancreatitis, 2 died and
37 were discharged between days 3 and 22. About 18-20% of them developed organ dysfunctions on day 7, the mean hospital stay was 13-15 days, the mean duration of pain was 5 days (2-8 days)(Bansal, 2011 [21546719]. These patients were divided into 2 groups, one of which received 1 gm of IV Vit C. No statistical difference was seen between the IVC group and the control group.

A group of 84 acute pancreatitis patients were divided into 2 groups, Treatment group (received IV Vit C 10 gm/day for 5 days) and Control group (IV Vit C 1 gm/day for 5 days). Treatment group improvement was statistically significant compared to the Control group, with disappearnace of abdominal pain within 55 hours, normalization of amylase within 79 hours. The hospital stay was 9.34 days for the Treatment group and 13.45 days for the Control group. Du, 2003 [14606098].

I recently took care of an acute pancreatitis patient (male, 82). After 5 hours of persistent epigrastric pain, he was admitted to the ICU. his amylase was 2800 and CT scan showed diffuse pancreatic secretions. He was given IV Vit C (IVC) 2 grams the first day in addition to conventional support care. IVC was increased to 12 grams on day 2 and maintained at 10 grm/day for 3 more days until discharge. His abdominal pain subsided gradually and disappeared on day 3, his amylase also reduced to normal. He was discharged on day 5 to home care.
